Amazon's description indicates that there is the option to "fill out the form on the back of the packaging and send to S4 to have Rx filled"... but what the item's description doesn't mention is that this option costs $91 (+ $10 shipping), to come out to >$100, per the back of the packaging.

From another pair that I have, I can say that the inserts work and are good. However, unless you have access to a cheap option for filling the Rx, the cost of the insert is negligible compared to the cost of filling the prescription, which is quite expensive.
